The evaluation forms are to be completed by parents, players and the Head Coach.

Team Managers:

1. Print and distribute a copy of the player form and the parent form to each family.
2. Print and provide the Head Coach with a copy of the Coach form. (Mandatory completion)
3. To ensure confidentiality, the forms are to be returned by each family in a sealed envelope to the Manager
4. Manager to then forward all returned forms, without reading them, to the Rep Co-ordinator for processing.

Please encourage the families on your team to complete the forms. These feedback forms are a very important tool that we use to try to enhance and to improve our Rep Programs. Your comments are encouraged and welcomed. After processing the forms the Rep Co-ordinator will meet and review the summarized results with the Head Coach. The Head Coach will not be given or shown copies of what any one family has submitted. Your submissions will be held in the strictest of confidence.
