HNS Risk Management Complaint Procedures

Please find attached a Memo from Don Matheson, VP of Operations/Risk
Management re: the complaint intake form and process for all HNS Risk
Management complaints.

The attached form must be completed in its entirety and forwarded to the HNS
office by fax at 454-3883 or email ppower@hockeynovascotia.ca . A copy of
the complaint intake form along with an official letter will be forwarded to
the President of the association in which the matter has to be dealt with.
It is the responsibility of the Minor Hockey Association to conduct the
investigation. The time line is a Maximum of 7 days before the results of
the investigation must be reported back to the HNS office.
